As our newest patient, we'd like to make your registration process as convenient as possible.You have two options for registering with us:
OPTION #1:
Simply arrive at our office 30 minutes before your scheduled appointment

Fill out the forms below and arrive 15 minutes before your scheduled visit
1. Review Important Reminders about Your Visit

Fill out and sign all downloadable forms before your appointment.

An original Arbitration Form will be given to you to sign at the time of your appointment.

Payments, in the form of Visa, MasterCard, Discover, American Express, check or cash, are expected at the time services are rendered.

There is a parking garage across the street from the office, located on Witmer & Shatto. Alternatively, metered parking is available on Wilshire and Witmer.

Bring your new patient forms with you when you come in for your first visits:
all new patient forms need to be complete before your first visit. Do not hesitate to discuss any questions you might have with us.
2. Print and Fill out the Patient Registration Form
This form tells us all the basic information about you we'll need to know like contact information and employer
3. Print and Fill out the Medical History Form
This form helps us understand the nature of your current condition:
4. Print and Fill out the Medical Release Form
At some point, we may wish to see a copy of your medical records, or another provider may request a copy of your medical records from us. This form authorizes us to get access to important medical information about you when necessary.
5. Print and Fill out the HIPAA Form
We are required by law to inform you of our privacy policies. This form both explains our policies and confirms that you have read and understood them.
6. Bring All Your Forms to the Office
Make sure to bring all your completed forms to the office with you. Once you arrive, we'll also provide you with a Patient Arbitration form.
